A mistake is only a mistake if you don't learn from it. The military calls it a debrief. The nuclear industry calls it. They use these tactics to teach team members "what not to do." Having a shared knowledge base and a company culture that allows failure (as long you learn from it) is key to preventing mistakes down the road. Develop a list of "lessons learned," add to it as your team members encounter issues, and then share it with everyone.
